Why this is not a ranked list
Search this term and you will find dozens of listicles ranking agencies. Almost all are written by agencies who placed themselves first, or by affiliates paid for placement.
We would do exactly the same thing if we wrote one, so we have not. What follows is the criteria we would apply if we were hiring on your behalf, and the questions that separate a specialist from a generalist quickly.

The last one matters more than it looks. An agency willing to tell you a term is not worth pursuing is an agency doing keyword research rather than selling hope. "minoxidil" at difficulty 71 against Mayo Clinic is not winnable, and anyone who says otherwise has not looked.

What genuine specialisation looks like
Category knowledge that shows up unprompted. An agency that works in telehealth knows that dermatology demand is ingredient-led, that peptides has no paid fallback, and that GLP-1 content lives or dies on the compounded versus branded distinction. That knowledge appears in the first conversation or it does not exist.
Clinical review as a process rather than a byline. Ask who signs off, what their credential is, and whether it is verifiable independently.
Link capability in health specifically. General link building and health-publisher link building are different disciplines with different costs, and an agency that cannot show the latter will quietly substitute the former.
And revenue measurement. Sessions and rankings are inputs. If an agency reports them as outcomes, they are not measuring the thing you care about.
