AI search optimisation for telehealth brands is the practice of structuring content so large language models retrieve and cite it when answering health and buying questions. It depends on the same authority signals as organic search, because assistants overwhelmingly cite sources that already rank.

The number most agencies will not show you
$32M
from organic search, per year
$19K
from AI search, per year
One client, over $100M in revenue, heavily cited across assistants.
One of our clients does over $100M in revenue. They earn roughly $32M a year from organic search. From AI search, despite being heavily cited across assistants, they earn about $19,000.
That is the honest state of the channel today, and we lead with it because the conclusion runs the opposite way to what you would expect.
AI search is not yet driving meaningful revenue for anyone. That is precisely why the cost of establishing a position is low right now, and why it will not stay low. The brands being cited today become the default answer tomorrow, and by the time the revenue arrives the citations will have been allocated.
02
AI search is a layer on top of SEO, not a replacement
A lot of agencies have this backwards and are selling generative engine optimisation as a standalone product. It is not one.
Assistants cite sources that already hold authority. The retrieval systems behind them lean heavily on the same signals that drive organic ranking: topical depth, corroborated authorship, structural clarity and links from sources with their own credibility. A brand with no organic authority does not get cited because a consultant restructured its headings.
The practical consequence is that you cannot buy AI visibility separately. You can buy work that improves both, or you can buy work that improves neither.
03
What actually drives citation
| Signal | Why assistants use it | How we build it |
|---|---|---|
| Standalone answer passages | Retrievers lift a paragraph without surrounding context | A 40 to 60 word direct answer block on every page |
| Complete topic coverage | Assistants prefer one comprehensive source to three partial ones | Absorbed sub-topics rather than thin separate pages |
| Structured comparison | Tables extract cleanly into an answer | A data table on every substantive page |
| Corroborated authorship | Cautious categories favour verifiable expertise | Named clinical reviewer with off-site standing |
| Crawlable HTML | Several AI crawlers do not execute JavaScript | Server-rendered pages, verified with curl |
The last row disqualifies more brands than the other four combined. A client-rendered site serves most AI crawlers an empty container. It is invisible to the channel regardless of how good the content is.

Health is a category where assistants are cautious, which helps you
Is peptide therapy safe, and who should I look at?
Evidence varies by peptide, and most are prescribed off-label, so a clinician should assess suitability first. Sources that discuss the research honestly, including the limits of it, are the ones cited here.
Sources
- 1Named brand, clinician-reviewed peptide guide
- 2Institutional health publisher
- 3Peer-reviewed journal summary
Assistants are noticeably more conservative in health than in most categories. They cite fewer sources, hedge more, and defer to institutional publishers.
That cuts both ways and mostly in your favour. It is harder to get cited, and a citation carries more weight with the reader because it reads as vetting rather than as a listing. In peptides and hormone categories especially, being one of the few brands an assistant will name is worth substantially more than a comparable organic position.
When I am asked if I know of anyone in the SEO space, without hesitation I point them to DTC SEO. They truly understand the power of authority content, onsite metrics, and strategic backlinking and has the network to put it into play.
