Half of their 337 keywords sit in the top three, which tells you the pages they have are fine. There are simply almost none of them.\n\nThis is a demand capture problem, not a quality problem, and it is the most fixable position in this entire teardown set.

"l carnitine injection" at 8,700 searches and difficulty 12 is their largest commercial opportunity and they sit at position 13 on it. Difficulty 12 means that position is available to whoever writes the better page.\n\nEverything else on the list is under 500 searches. The TRT demand that matters, and our own category page documents it, sits on terms like "low testosterone treatment" at 3,100 and difficulty 19, and "trt online" at 2,100 with a $7.00 CPC. Marek holds neither.

Where the flank is open
This is less a teardown than a demonstration of what the TRT category looks like when nobody builds for search.\n\nThe symptom layer is untouched. Men do not search for hormone clinics, they search for what low energy, poor sleep and declining strength mean, and then they search what to do about it. That path is the entire funnel in this category and 337 keywords cannot cover it.\n\nThe protocol layer is the second gap. Ancillary medications, injection versus cream, lab panel interpretation and what a follow-up looks like are all high intent queries from people already committed to treating, and they are cheap to rank for because most brands are too cautious to publish them properly.\n\nThe third is that a brand with real audience recognition and no search position is paying twice: once to build the awareness, and again when the demand it created resolves to a competitor's page.
